Online Make Up Exam Policy

New expanded services for Fall 2016!

Beginning Fall 2016, we are now able to accept makeup exam information via e-mail. Makeup exam information can be submitted to a new e-mail address (available soon).

Canvas is recommended as our preferred campus solution for online examinations. If you would like training on how to set up a Canvas site for your course, please contact Academic Technology for assistance. If you already have a Canvas site and need help setting up an exam, please scroll down for additional directions.

Please allow 2 business days for processing time when submitting e-mails. For faster processing, you may still drop off exam information in-person (preferred).

Our updated procedures are as follows:

    1. Download and fill out a Makeup Exam Info Sheet for each exam, one per student.
      • All fields are necessary. If any information is missing, we may contact you before working with your student, which can delay processing.
      • Please include the Canvas access code, otherwise we will not be able to administer the exam!
    2. Set up the exam on Canvas. (scroll down for instructions if needed)
      • Please make sure to allow at least 2 days for processing when setting the exam availability. In urgent cases where that is not possible, we will need to speak with you in-person to ensure that we have all the correct information to administer the exam properly.
      • Please note: We are not responsible for technical issues within the Canvas software or your Canvas site.
    3. E-mail us at makeup-exams@sjsu.edu with:
      • A completed Makeup Exam Info Sheet
      • A list of additional resources that students may use (if any)
      • Any additional software requirements for your exam (if new software, may delay processing)
      • Any additional instructions that our staff may need
    4. Once the exam materials are delivered, allow 2 business days for processing.
      • Our office will send you a confirmation e-mail. At that point, please instruct your student to e-mail our office to make an appointment to take the exam within your specified time window.
      • Student appointments for makeup exams can be made at testing-office@sjsu.edu.
      • You will receive a notification in the Canvas software once your student has completed the exam.

 

To set up a makeup exam on Canvas:

    1. Request a Canvas account, if you do not have one already.

    2. Login to Canvas

    3. To set up for a multiple choice exam:

      • From the top of the welcome page, click on Courses and select the correct courses
      • On the left hand side, click on Quizzes
      • On the pull down menu, choose Quiz type as Graded Quiz
      • Click on the check box for Shuffle questions if more than one student is taking the same test
      • Click on the check box for Time Limit and enter the time limit
      • Click on the check box for Restrict this quiz and enter an access code (This is to ensure no students have access to the test and the test can only be accessed by Testing Office personnel)
      • For closed book exams, click on the check box for Require Respondus LockDown Browser
      • Click on the check box for Show one question at a time
      • Select the student from your class
      • Select the date and time for the available from (Start day student can access the test)
      • Select the date and time for the until (Last day student can access the test)
      • Select the Question Tab and enter all questions and responses
      • Continue to 5 and 6 below
    4. To set up a short answer or essay type exam:

      • From the top of the welcome page, click on Courses and select the correct courses
      • On the left hand side, click on Quizzes
      • On the pull down menu, select Quiz type as Graded Quiz
      • Click on the check box for Shuffle questions if more than one student is taking the same test
      • Click on the check box for Time Limit and enter the time limit
      • Click on the check box for Restrict this quiz and enter an access code (This is to ensure no students have access to the test and the test can only be accessed by Testing Office personnel)

The Testing Office will provide the following assurances:

    1. We provide a safe, comfortable, distraction free environment for students to take their tests.

    2. We will report any irregularities to the instructor, including students cheating, failing to follow directions, or not abiding by the test time limits. 

    3. We will keep all test information and passwords in a secure location.
