Key Instructions

The key form (General Purpose Answer Key) serves dual purposes. One part of the form serves as the key for the test; the other provides information necessary to process the test. These instructions need to be followed carefully. A #2 pencil only must be used to fill out the key. All marks should be dark and completely fill the bubbles. Be aware that the Testing Office cannot be responsible for errors or incomplete key form(s).

    1. Item 1, Name: Fill in the area for your Last Name, one letter to a box. Then, grid in the corresponding letters under each box.
    2. Item 2, Course: This section is optional. If used, all the students' answer sheets should match your key in this area.
    3. Item 3, Test Form and Number: This section is optional. If used, all the students' answer sheets should match your key in these areas.
    4. Item 4, Sorting Options: Results can be printed either by the students' last names, their San Jose State University Identification Number (SJSU ID), or both. In addition, either students' names or their SJSU ID's can be sorted. In the case of names, the last names of the students will be alphabetically sorted and then printed. The SJSU ID will be sorted and printed in ascending order. The instructor can choose the sorting option for his/her needs. Choose a sorting option by filling out the grid corresponding to the options listed.
    5. Item 5, Do You Want Choice Analysis Printed? If you answer yes, an analysis report on the response choices of each question for the batch of answer sheets are scored. If no, this analysis report will be omitted.
    6. Item 6, No. of Test Questions including Subtest, Total Questions and Scoring Options:
      • With subtest: If you have subtests, that is, if you wish to divide the test into two or more parts, you may indicate how many questions comprise each subtest. Do this by marking the appropriate numbers under each subtest column. The total questions in each subtest must equal the total number of questions.
      • No subtest: Just enter the total number of questions for the test. (Note: the total number of questions is the last assigned question number even if you have omitted some questions on this particular test. Please contact Testing Office for further clarification.)
      • Scoring options: There are six different scoring formulas available for scoring the test. If you have subtest, different scoring formulas can be used to score different parts of the test. To select a scoring formula for a subtest, use the Subtest Section to select the scoring option for each subtest. If you do not have a subtest, just use the Total Section for your selection. *see below
    7. Answer Key: On the right side of the sheet, mark the correct answer for each question. Only one correct answer per question is allowed. If omitting an item, leave that question unmarked. The scoring program omits any question with more than one answer choice.
